Mono-Therapy versus Poly-Therapy: Ten Years Indian Experience on Various Seizure Disorders  [PDF]

Abstract: Objectives: To find out prescription patterns and seizure freedom with different mono-, duo- and poly-therapies used in various seizure disorders in Indian setting. Material and Methods: Subjects with seizures, not responding to first antiepileptic drug, were evaluated prospectively for a period of 6 months. Patients on various dual antiepileptic drugs (AEDs) prescriptions were prospectively analyzed for 1) the pattern and frequency of different duo-therapies; 2) etiological profile of patients in duo-therapy prescription; and 3) frequency of seizure freedom on various duo-therapy groups. Results: Among 2542 patients, 293 (11.5%) lost in follow-up and thus, 2249 (88.5%) were followed. 1324 (58.9%) had seizure freedom on mono-therapy, 532 (23.7%) required duo-therapy and only 45 (2%) were better controlled on poly-therapy. Among the subjects, who were on mono-therapy, Carbamazepine/Oxcarbazepine was the most commonly prescribed in 1285 (50.55%) patients as first drug followed by Valproate compound and Phenytoin. The most common duo-therapy used was combination of Valproate with Lamotregine, followed by Phenytoin and Phenobarbitone. Other mono-therapy and combinations are given in this paper. Conclusions: Duo-therapy was required and found to be effective in 23.7% of Indian patients with epilepsy. Selection of appropriate two drug combination is based on individual approach and overall clinical profile of patient.

Abstract: The application of mathematical morphology to imageprocessing and analysis has initiated a new approach forsolving a number of problems in the image enhancement field.So, we have used morphological operators to detect and extractthe background of images. Various operators used are erosion,dilation, opening and closing. After the background is extracted, contrast of bad lightning image is improved by the application of operators based on Weber’s law notion. The operators used are block analysis and opening by reconstruction. In this paper, we will present the result of images on applying these operators.
Atypical teratoid/rhabdoid tumors of central nervous system

Abstract: Aims and Objectives: To review the clinical features and current understanding of the epidemiology, biology and management of pediatric atypical teratoid/rhabdoid tumors and analyzing the different treatment modalities. Materials and Methods: The MEDLINE database, bibliographies of selected articles and current English-language texts on the subject were reviewed. A Pubmed search was made with keywords pediatric atypical teratoid/rhabdoid tumors, intracranial, surgery, chemotherapy and radiotherapy. Most recent articles and also significant older articles having all above said words were selected and their results were reviewed in detail. Results: Atypical teratoid/rhabdoid tumors (AT/RT) of the central nervous system (CNS) most frequently diagnosed in smaller than 3 years of age and incidence is 1-2% of all brain tumors in children. 63% of the AT/RT of the CNS is seen in infra-tentorial compartment, there are no precise imaging features that differentiate AT/RT from the other posterior fossa tumor. The "rhabdoid" cells are characteristic on cytopathology. It has been established now that CNS, AT/RT often show deletion of the long arm of chromosome 22q11.2. The initial treatment for most children with AT/RT is surgical with and without cerebrospinal fluid diversionary procedure. Children with less than 3 years of age offered chemotherapy but in older children radiotherapy is given in addition.
Light bending and perihelion precession: A unified approach

Abstract: The standard General Relativity results for precession of particle orbits and for bending of null rays are derived as special cases of perturbation of a quantity that is conserved in Newtonian physics, the Runge-Lenz vector. First this method is applied to give a derivation of these General Relativity effects for the case of the spherically symmetric Schwarzschild geometry. Then the lowest order correction due to an angular momentum of the central body is considered. The results obtained are well known, but the method used is rather more efficient than that found in the standard texts, and it provides a good occasion to use the Runge-Lenz vector beyond its standard applications in Newtonian physics.

Abstract: An ad-hoc network is a collection of wireless mobile hosts forming a temporary network without the aid of any standalone infrastructure or centralized administration. Routing in Ad hoc networks is a challenging problem because nodes are mobile and links are continuously being created and broken.In this model we not only improves the reputation of the network but also provide a routing approach for reliable data transmission and also avoid the loop occurs in the communication. The mobile network is the dynamicnetwork that provides the solution for the inclusion and exclusion of dynamic nodes in the network. AODV and DSR are the two most popular routing protocols for ad-hoc network that we discussed here. In this paper we describe the way to find the node having packet loss and to perform the elimination of node from the network withoutperforming the rerouting and provide the reliable data transfer over the network. In this paper, we design and evaluate cooperative caching techniques to efficiently support data access in the ad-hoc network.
Vulnerability Management for an Enterprise Resource Planning System

Abstract: Enterprise resource planning (ERP) systems are commonly used in technical educational institutions(TEIs). ERP systems should continue providing services to its users irrespective of the level of failure. There could be many types of failures in the ERP systems. There are different types of measures or characteristics that can be defined for ERP systems to handle the levels of failure. Here in this paper, various types of failure levels are identified along with various characteristics which are concerned with those failures. The relation between all these is summarized. The disruptions causing vulnerabilities in TEIs are identified .A vulnerability management cycle has been suggested along with many commercial and open source vulnerability management tools. The paper also highlights the importance of resiliency in ERP systems in TEIs.
